Objectives: Chronic low-back pain (CLBP) is burdensome and costly, and a common condition for which adults use integrative therapies. The effectiveness of multidisciplinary integrative approaches has not been well studied. The purpose of this observational study was to compare characteristics and outcomes of CLBP patients treated at the Osher Clinical Center (OCC) versus other clinics at Brigham and Women's Hospital. Design: Observational comparative effectiveness study. Setting: Tertiary care hospital. Subjects: Patients >= 21 years with 3+ months of CLBP or 6+ months of intermittent low-back pain. Intervention: All patients were observed for 12 months. OCC patients received care at the integrative clinic (7.3 visits on average over 13 weeks); non-OCC patients received usual care at other clinics of the same hospital. Outcome measures: Primary outcomes: change from baseline to 6 months in functional status (Roland Disability Questionnaire [RDQ]) and bothersomeness of pain (BOP). Secondary outcomes: change in RDQ and BOP at 3 and 12 months, percentages of patients with clinically meaningful (>= 30%) improvements. Results: One hundred fifty-six OCC and 153 non-OCC participants were enrolled; follow-up was 90.4 and 98.0%, respectively, at 12 months. There were substantial differences in baseline characteristics between groups. For RDQ, the adjusted mean group difference was nonsignificant at 6 months; for BOP, the differences were significant, but clinically small. At 12 months, the observed benefit on RDQ was significant and clinically meaningful; for BOP, there were significant, but clinically small differences. Percentages of patients with >= 30% improvements in RDQ were significantly greater in the OCC group only at 12 months, and both 6 and 12 months for BOP. Conclusions: Baseline characteristics can differ between those who select different sources of healthcare for CLBP. While benefits seen in the OCC versus non-OCC clinics were not large, further evaluation through randomized trials might be warranted to provide a more definitive evaluation.

The symptoms of borderline personality disorder (BPD) have been characterized as deficits in mindfulness. Mindfulness can be defined as nonjudgmental, present-centered awareness. The present study investigates the theory that, consistent with this conceptualization, the extent to which acting with awareness predicts reduced BPD features and related dysfunction depends upon levels of nonjudgment. In a sample of 223 undergraduates, we calculated the interaction between awareness-based and nonjudging-based mindfulness skills using subscales of the Five Facet Mindfulness Questionnaire. Regression analyses demonstrated a significant effect of the interaction on several difficulties that are common in BPD: problems with relationships, emotion-related impulsivity, and anger rumination. For acting with awareness to benefit individuals with these difficulties, a less judgmental stance toward internal experiences may be necessary. These findings have significant treatment implications and demonstrate the importance of assessing mindfulness as a multifaceted, synergistic construct.

<p>ABSTRACTIn order to identify potential training to enhance comprehensive soldier fitness, this analysis searched MEDLINE via PubMed and elsewhere for 33 reasonably significant modalities, screening over 11,500 articles for relevance regarding soldier resilience. Evaluation of modalities that are exclusively educational or cognitive/behavioral in nature is deferred. Using the volume and quality of research over 40 parameters distributed among the five domains of resilience (physical, emotional, spiritual, social, and family life), these data allow culling of most of the meditative modalities and discrimination among the remaining techniques. The resulting order of merit is Transcendental Meditation, mindfulness, and progressive muscle relaxation. Transcendental Meditation, mindfulness, and progressive muscle relaxation, in that order, have the most supporting data. Fortuitously, they also represent a cross section of the domain of techniques regarded as meditation, stress management, or relaxation, with three very different mechanisms of action. They are suitable potential options for improving soldier resilience.</p>

Participants self-selected into MBSR or CBSR courses taught at different times. There were no initial differences between the MBSR and CBSR subjects on demographics, including age, gender, education, and income. Intervention: MBSR was an 8-week course using meditation, gentle yoga, and body scanning exercises to increase mindfulness. CBSR was an 8-week course using cognitive and behavioral techniques to change thinking and reduce distress. Design: Perceived stress, depression, psychological well-being, neuroticism, binge eating, energy, pain, and mindfulness were assessed before and after each course. Pre-post scores for each intervention were compared by using paired t tests. Pre-post scores across interventions were compared by using a general linear model with repeated measures. Settings/Locations: Weekly meetings for both courses were held in a large room on a university medical center campus. Results: MBSR subjects improved on all eight outcomes, with all of the differences being significant. CBSR subjects improved on six of eight outcomes, with significant improvements on well-being, perceived stress, and depression. Multivariate analyses showed that the MBSR subjects had better outcomes across all variables, when compared with the CBSR subjects. Univariate analyses showed that MBSR subjects had better outcomes with regard to mindfulness, energy, pain, and a trend for binge eating. Conclusions: While MBSR and CBSR may both be effective in reducing perceived stress and depression, MBSR may be more effective in increasing mindfulness and energy and reducing pain. Future studies should continue to examine the differential effects of cognitive behavioral and mindfulness-based interventions and attempt to explain the reasons for the differences.

While much recent scholarship on Tantra has tended to distance it from the world of pragmatic ritual healing, mediumship and spirit-possession, and treat it primarily as an elevated mode of pursuit of enlightened consciousness, Frederick Smith’s recent book The Self Possessed is the most comprehensive presentation so far of the reverse position, that all of these modes of interaction with the divine, whether to do with healing, sorcery or spiritual liberation, share common assumptions and a common idiom. Here, and in other recent writing, Smith presents the idea of entry, pervasion or possession (āveśa) as a fundamental trope in Indic thought, encompassing everything from conception (seen as the individual jīva taking over possession of the embryo) to Tantric ritual, the temporary occupation of another body and malevolent spirit-attack. If this is true, however, does this suggest that we are applying the wrong set of categories to understand Indic and perhaps also other religious traditions? This article sketches an alternative mode of looking at the field of “possession,” broadly defined, and explores some of its implications.

Enhancing Quality of Life (QOL) has long been an explicit or implicit goal for individuals, communities, nations, and the world. But defining QOL and measuring progress toward meeting this goal have been elusive. Diverse “objective” and “subjective” indicators across a range of disciplines and scales, and recent work on subjective well-being (SWB) surveys and the psychology of happiness have spurred interest. Drawing from multiple disciplines, we present an integrative definition of QOL that combines measures of human needs with subjective well-being or happiness. QOL is proposed as a multi-scale, multi-dimensional concept that contains interacting objective and subjective elements. We relate QOL to the opportunities that are provided to meet human needs in the forms of built, human, social and natural capital (in addition to time) and the policy options that are available to enhance these opportunities. Issues related to defining, measuring, and scaling these concepts are discussed, and a research agenda is elaborated. Policy implications include strategies for investing in opportunities to maximize QOL enhancement at the individual, community, and national scales.

Working memory (WM) comprises operations whose coordinated action contributes to our ability to maintain focus on goal-relevant information in the presence of distraction. The present study investigated the nature of distraction upon the neural correlates of WM maintenance operations by presenting task-irrelevant distracters during the interval between the memoranda and probes of a delayed-response WM task. The study used a region of interest (ROIs) approach to investigate the role of anterior (e.g., lateral and medial prefrontal cortex--PFC) and posterior (e.g., parietal and fusiform cortices) brain regions that have been previously associated with WM operations. Behavioral results showed that distracters that were confusable with the memorandum impaired WM performance, compared to either the presence of non-confusable distracters or to the absence of distracters. These different levels of distraction led to differences in the regional patterns of delay interval activity measured with event-related functional magnetic resonance imaging (fMRI). In the anterior ROIs, dorsolateral PFC activation was associated with WM encoding and maintenance, and in maintaining a preparatory state, and ventrolateral PFC activation was associated with the inhibition of distraction. In the posterior ROIs, activation of the posterior parietal and fusiform cortices was associated with WM and perceptual processing, respectively. These findings provide novel evidence concerning the neural systems mediating the cognitive and behavioral responses during distraction, and places frontal cortex at the top of the hierarchy of the neural systems responsible for cognitive control.

Important features of the self-concept can be located outside of the individual and inside close or related others. The authors use this insight to reinterpret data previously said to support the empathy-altruism model of helping, which asserts that empathic concern for another results in selflessness and true altruism. That is, they argue that the conditions that lead to empathic concern also lead to a greater sense of self-other overlap, raising the possibility that helping under these conditions is not selfless but is also directed toward the self. In 3 studies, the impact of empathic concern on willingness to help was eliminated when oneness--a measure of perceived self-other overlap--was considered. Path analyses revealed further that empathic concern increased helping only through its relation to perceived oneness, thereby throwing the empathy-altruism model into question. The authors suggest that empathic concern affects helping primarily as an emotional signal of oneness.

In a world where information technology is both important and imperfect, organizations and individuals are faced with the ongoing challenge of determining how to use complex, fragile systems in dynamic contexts to achieve reliable outcomes. While reliability is a central concern of information systems practitioners at many levels, there has been limited consideration in information systems scholarship of how firms and individuals create, manage, and use technology to attain reliability. We propose that examining how individuals and organizations use information systems to reliably perform work will increase both the richness and relevance of IS research. Drawing from studies of individual and organizational cognition, we examine the concept of mindfulness as a theoretical foundation for explaining efforts to achieve individual and organizational reliability in the face of complex technologies and surprising environments. We then consider a variety of implications of mindfulness theories of reliability in the form of alternative interpretations of existing knowledge and new directions for inquiry in the areas of IS operations, design, and management.

This article examines how social emotional learning contributes to bullying prevention efforts in schools. Bullying behavior is impacted by multiple levels of the social-ecology of schools. Social emotional learning (SEL) is a structured way to improve a wide range of students' social and emotional competencies and impact bullying at the individual and peer levels of the school social-ecology. SEL has been shown to be an effective component in comprehensive bullying prevention interventions and other interventions targeting problems such as substance abuse. SEL programs have also been shown to improve student skills, reduce problem behaviors, and increase academic achievement. This article discusses how skills taught in SEL programs contribute to bullying prevention and shows the research links between SEL skills and bullying. Specific suggestions are provided for teachers of ways to support student skill acquisition and strengthen the gains provided by SEL programs.

This article describes the results of a national survey pertaining to school psychologists' knowledge and use of evidence-based, social-emotional learning (SEL) interventions. For the study, 331 school psychologists responded to a survey that listed (a) techniques for identifying SEL interventions, (b) 16 SEL programs that have been identified by more than one source as having strong evidence for their effectiveness, and (c) factors that school psychologists may use for deciding on a program to use in their schools. Participants in the survey were asked to rate their opinions about selecting and using SEL interventions, as well as their knowledge and experience with various SEL programs that have received much research attention. Results of the survey indicated that school psychologists have limited awareness of the majority of published, evidence-based SEL programs. These results are of interest to school psychologists and other school personnel who make decisions about purchasing and implementing SEL programs. Implications for training and practice are discussed. (Contains 3 tables.)

Social-emotional (SE) skills in the early developmental years of children influence outcomes in psychological, behavioral, and learning domains. The adult ratings of a child's SE skills can be influenced by sex stereotypes. These rating differences could lead to differential conclusions about developmental progress or risk. To ensure that differences between boys and girls in SE skills are accurate, validity evidence should support that differences are not based on such issues as stereotypes influencing ratings. Differential item functioning (DIF) analysis allows for the assessment of group differences in item responses while controlling for ability. This study utilized a new multilevel Mantel-Haenszel (MMH) DIF procedure to examine sex differences in item responses for examiner ratings of children's SE skills on the Brigance Inventory of Early Development III SE scale. Of 50 items examined, 4 were identified as large DIF items. The scores do not appear to be influenced by item-level rating distortions based on sex stereotypes.

Humanity has reached a defining moment in that unless it makes a transition to live sustainably and in harmony with nature, it will put at risk its own future and that of the Earth’s ecology and the diversity of the biological resources. Humans cannot exist without the biodiversity inherent in a healthy environment. Natural diversity comprises a web of interdependent ecosystems, species, populations, and genes that, in combination, create an enabling environment for the provision of nutritious food, clean water, and fresh air, essential for a sustaining environment.

This study sought to examine tai-chi as an anxiety-reducing method to increase self-efficacy and improve performance among baccalaureate nursing students experiencing a patient care simulation. The sample included 63 nursing students enrolled in a traditional or second-degree nursing program. The study used a randomized control group pretest-posttest design. Students in the experimental group experienced significant reductions in cognitive and somatic anxiety, along with an increase in self-efficacy and improved performance. Tai-chi can be an effective technique to reduce anxiety and improve self-efficacy and performance among nursing students in simulations.

Background: School-based social-emotional and character development (SECD) programs can influence not only SECD but also academic-related outcomes. This study evaluated the impact of one SECD program, Positive Action (PA), on educational outcomes among low-income, urban youth. Methods: The longitudinal study used a matched-pair, cluster-randomized controlled design. Student-reported disaffection with learning and academic grades, and teacher ratings of academic ability and motivation were assessed for a cohort followed from grades 3 to 8. Aggregate school records were used to assess standardized test performance (for entire school, cohort, and demographic subgroups) and absenteeism (entire school). Multilevel growth-curve analyses tested program effects. Results: PA significantly improved growth in academic motivation and mitigated disaffection with learning. There was a positive impact of PA on absenteeism and marginally significant impact on math performance of all students. There were favorable program effects on reading for African American boys and cohort students transitioning between grades 7 and 8, and on math for girls and low-income students. Conclusions: A school-based SECD program was found to influence academic outcomes among students living in low-income, urban communities. Future research should examine mechanisms by which changes in SECD influence changes in academic outcomes.

Ecosystems are capital assets: When properly managed, they yield a flow of vital goods and services. Relative to other forms of capital, however, ecosystems are poorly understood, scarcely monitored, and--in many important cases--undergoing rapid degradation. Sensitivity to fairness may influence whether individuals choose to engage in acts that are mutually beneficial, selfish, altruistic, or spiteful. In a series of three experiments, chimpanzees (Pan troglodytes) could pull a rope to access out-of-reach food while concomitantly pulling another piece of food further away. In the first study, they could make a choice that solely benefited themselves (selfishness), or both themselves and another chimpanzee (mutualism). In the next two experiments, they could choose between providing food solely for another chimpanzee (altruism), or for neither while preventing the other chimpanzee from receiving a benefit (spite). The main result across all studies was that chimpanzees made their choices based solely on personal gain, with no regard for the outcomes of a conspecific. These results raise questions about the origins of human cooperative behaviour.
