Although the efficacy of behavioral interventions for migraine (e.g., relaxation training, stress management, cognitive-behavioral therapy, biofeedback) is well established, other behavioral interventions that have shown efficacy for other conditions are being adapted to treat migraine. This paper reviews the literature to date on acceptance and commitment therapy (ACT), mindfulness-based interventions, and behavioral interventions for common migraine comorbidities. ACT and mindfulness interventions prioritize the outcome of improved functioning above headache reduction and have demonstrated efficacy for chronic pain broadly. These emerging behavioral therapies show considerable promise for improving outcomes of migraine patients, particularly in reducing headache-related disability and affective distress, but efficacy to date is limited by small trials, short follow-up periods, and a need for comparison or integration with established pharmacologic and behavioral migraine treatments.

OBJECTIVE:The purpose of the current study was to examine the relationship between the facets of trait mindfulness with psychological and physical health while controlling for health behaviors in college students. PARTICIPANTS: 310 students from a small, private college in the Northeastern United States. OUTCOME MEASURES: Students completed self-report measures, including the Perceived Stress Scale (PSS-14), the Five Facet Mindfulness Questionnaire (FFMQ), Medical Outcomes Short-Form Health Survey (SF-36), and the Rutgers Alcohol Problems Index (RAPI). RESULTS: After controlling for other health behaviors, the observation facet of mindfulness was negatively associated with physical health. Both acting with awareness and nonjudging facets were positively associated with emotional well-being. For social functioning, nonjudging was a significant positive facet for this domain of health. CONCLUSIONS: Tailoring mindfulness-based interventions to enhance these facets may be beneficial to young adults.

<p>In this commentary I discuss the integration of mindful procedures in cognitive therapy of generalized anxiety disorder (CAD) and attempt to answer questions concerning the effects of mindfulness on information processing and on mechanisms purported to maintain CAD in the meta-cognitive model of this disorder. Different techniques that promote mindfulness can be identified, including mindfulness meditation and attention training. These techniques are intended to disrupt repetitive styles of dysfunctional thinking. I argue that the effect of mindfulness strategies on information processing in emotional disorder can be conceptualized in meta-cognitive terms as (a) activating a meta-cognitive mode of processing; (b) disconnecting the influence of maladaptive beliefs on processing; (c) strengthening flexible responding to threat; and (d) strengthening meta-cognitive plans for controlling cognition. Although mindfulness meditation may have general treatment applications, the meta-cognitive model of CAD suggests caution in using this treatment in CAD. It is unclear which dimension of worry should be targeted, and mindfulness meditation does not contain information that can lead to unambiguous disconfirmation of erroneous beliefs about worry.</p>

This study compared two techniques, detached mindfulness (DM) and thought evaluation (TE), for dealing with negative thoughts that are drawn from different treatment modalities. Twelve participants with high social anxiety practised each technique in a cross-over repeated measures design before giving a speech. It was predicted that each technique would be advantageous, but that DM would be superior to TE overall. Results showed that both techniques improved anxiety scores. DM led to reductions in the observer-perspective, negative beliefs, and anticipatory processing as well. The overall change attributed to DM was greater than that attributed to TE. The results also suggested that combining these techniques in certain ways might prove disadvantageous. Results are discussed in terms of the potential clinical implications.

Those with high baseline stress levels are more likely to develop mild cognitive impairment (MCI) and Alzheimer's Disease (AD). While meditation may reduce stress and alter the hippocampus and default mode network (DMN), little is known about its impact in these populations. Our objective was to conduct a "proof of concept" trial to determine whether Mindfulness Based Stress Reduction (MBSR) would improve DMN connectivity and reduce hippocampal atrophy among adults with MCI. 14 adults with MCI were randomized to MBSR vs. usual care and underwent resting state fMRI at baseline and follow-up. Seed based functional connectivity was applied using posterior cingulate cortex as seed. Brain morphometry analyses were performed using FreeSurfer. The results showed that after the intervention, MBSR participants had increased functional connectivity between the posterior cingulate cortex and bilateral medial prefrontal cortex and left hippocampus compared to controls. In addition, MBSR participants had trends of less bilateral hippocampal volume atrophy than control participants. These preliminary results indicate that in adults with MCI, MBSR may have a positive impact on the regions of the brain most related to MCI and AD. Further research with larger sample sizes and longer-follow-up are needed to further investigate the results from this pilot study.

A meta-cognitive classification and analysis of factors contributing to the development of problematic worry is presented. Dimensions of meta-beliefs, meta-worry, cognitive consciousness, and strategies can be distinguished. A cognitive model of Generalized Anxiety Disorder is advanced based on this framework in which GAD results from an interaction between the motivated use of worry as a coping strategy, negative appraisal of worry, and worry control attempts. These factors result from combinations of dysfunctional meta-beliefs and contribute to subjectively diminished cognitive control. The model presents new implications for a cognitive therapy of GAD, and these are illustrated with a single case treatment study.

Metacognitive Therapy (MCT) for depression is a formulation-driven treatment grounded in the Wells and Matthews (Attention and emotion: A clinical perspective, 1994) self-regulatory model. Unlike traditional CBT it does not focus on challenging the content of depressive thoughts or on increasing mastery and pleasure. Instead it focuses on reducing unhelpful cognitive processes and facilitates metacognitive modes of processing. MCT enables patients to interrupt rumination, reduce unhelpful self-monitoring tendencies, and establish more adaptive styles of responding to thoughts and feelings. An important component of treatment is modification of positive and negative metacognitive beliefs about rumination. MCT was evaluated in 6–8 sessions of up to 1 h each across 4 patients with recurrent and/or chronic major depressive disorder. A non-concurrent multiple-baseline with follow-up at 3 and 6 months was used. Patients were randomly allocated to different length baselines and outcomes were assessed via self-report and assessor ratings. Treatment was associated with large and clinically significant improvements in depressive symptoms, rumination and metacognitive beliefs and gains were maintained over follow-up. The small number of cases limits generalisability but continued evaluation of this new brief treatment is clearly indicated.

OBJECTIVE:To examine the relationship between mindfulness and alcohol problems in college students, as well as the role of stress as a mediator in this relationship. PARTICIPANTS: Participants were 310 students from a small, private college in the Northeast. METHODS: Students completed self-report measures, including the Perceived Stress Scale, the Five Facet Mindfulness Questionnaire, and the Rutgers Alcohol Problems Index. RESULTS: Mindfulness was negatively correlated with alcohol problems and stress, whereas stress positively correlated with alcohol problems. Results implicated stress as fully mediating the relationship between mindfulness and alcohol problems. Alcohol problems were negatively correlated with the Acting With Awareness and Describing Experience facets of mindfulness. CONCLUSION: Mindfulness-based stress reduction or other mindfulness programs may be useful in decreasing alcohol problems on college campuses via the effects on stress.

Objective: The purpose of the study is to determine if an updated online evidence-based educational programme delivered through Facebook is effective in improving the knowledge, skills, and self-efficacy of patients with arthritis in relation to evidence-based self-management rehabilitation interventions for osteoarthritis (OA) and rheumatoid arthritis (RA). Methods: Adult patients (>18 years old) with self-reported OA or RA were eligible for the study. One-hundred-and-ten participants were recruited from the general public and different arthritis patient organizations throughout Canada. Eleven participants were selected to participate in focus groups to select effective self-management strategies for OA and RA according to level of implementation burden. Ninety-nine participants were then selected to participate in the online Facebook intervention which included a group' web page providing case-based video clips on how to apply the selected self-management interventions. Over a three-month period participants were asked to complete three online questionnaires regarding their previous knowledge, intention to use/actual use of the self-management strategies, self-efficacy and confidence in managing their condition. Results: Knowledge acquisition scores improved among OA and RA participants with a mean difference of 1.8 (p < 0.01) when compared from baseline to immediate post-intervention. At three months post-intervention, almost all self-management strategies were successful with participants following through on their intention to use the self-management strategies; however, statistically significant results were only demonstrated for aquatic jogging' (p < 0.05) and yoga' (p < 0.05) among OA participants, and aquatic therapy' (p < 0.01) among RA participants. Self-efficacy was maintained from immediate post-intervention to three months follow-up, and confidence improved as the study progressed. Conclusions: This online programme can provide patient organization representatives with the opportunity to learn about and integrate evidence-based self-management strategies for OA and RA in their daily lives, to increase their awareness of useful community resources, and support their efforts to disseminate the information to others with arthritis.

This conceptual article presents a review of the research concerning the stress level of principals over the past three decades, with emphasis on the occupational stress that principals encounter because of heightened accountability and expectations for student achievement. Mindfulness meditation, as a stress management intervention, provides the theoretical background for this article; the scientific evidence concerning benefits of mindfulness meditations are reviewed. Finally, the author presents suggestions for the prevention and reduction of stress for principals.
